How to Access the SIS Patient Portal

You will receive an email and/or text message from SIS (Surgical Information Systems) shortly after registration, and then will be reminded of your upcoming appointment ahead of time:

• Two weeks before your procedure, you will receive a text reminder of the upcoming procedure.
• The day before your procedure, you will receive a text reminder of the upcoming procedure.
• You will receive a phone call to confirm your arrival time the business day (no weekends/holidays) prior.

Follow these steps to access the SIS Patient Portal

1. Check Your Messages
   Look for a text or email from SIS. These messages will include a secure link to the Patient Portal.

2. Click the Link
   Tap or click the link provided in the message to open the SIS Patient Portal.

3. Enter Your Information
   When prompted, enter the following exactly as it appears in your medical records:

   • First Name

   • Last Name

   • Date of Birth (DOB)

   Tip: Be sure to use your legal name (not a nickname) to avoid login issues.

4. Having Trouble Logging In?
   If you can’t access the portal:

   • Double-check your spelling and DOB.

   • Make sure you’re using your full legal name.

   • Still having trouble? Call the center directly for assistance.
